Payment for Services
Payment is expected at the time services are provided. If you have dental insurance, we will provide an estimate of the expected out-of-pocket expenses; deductibles and co-payment portion will be due at the time of your appointment. We accept cash, checks, Visa, MasterCard, Discover and American Express. Any unpaid balance over 120 days will be considered delinquent and turned over to a collection agency. Fees may apply.

Insurance
At Seven Corners Dental Care, our goal is to maximize your insurance benefits and make any remaining balance affordable. As a courtesy to our patients, we will verify dental insurance benefits and submit claims on your behalf. We will do our best to estimate the your insurance portion and co-payment. This may or may not be what the insurance company will actually pay. To facilitate benefit verification and claim submission, we ask that you provide us with accurate and up-to-date subscriber and insurance information.
Remember your insurance coverage is a contract between you, your employer, and your insurance company. Benefits and coverage vary significantly from plan to plan. Insurance is not designed to provide 100% benefit, but rather is meant to assist you with the cost of dental care.

If you cannot make your appointment, we kindly ask you give a 24 hour cancellation notice. We reserve the right to charge for a broken appontment.
